The Oakland Schools Transition Network is hosting a Transition Conference on Tuesday, November 18, 2011 from 8:30 a.m. – 2:30 p.m. at the Troy Community Center (3179 Livernois, Troy, MI 48083).

This event will focus on resources for individuals with behavioral health needs. Feature sessions include:

• Planning for Career
• Getting My Own Address
• Parent & Peer Supports
